When evaluating documentation and determining appropriate accommodations, several key factors are considered. Below is the list of documentation requirements.

  • Issued by a qualified professional (e.g., medical doctor, psychologist, licensed clinical social worker) with appropriate credentials and licensing.
  • Clearly dated, legible, and includes contact and licensing information to verify authenticity.
  • Preferably provided directly by the qualified professional. If submitted by a third party, it must identify the qualified professional.
Diagnosis
  • Clearly states the diagnosed disability or condition.
  • Based on objective diagnostic criteria (e.g., medical exams, clinical evaluations).
  • Includes complete diagnostic assessments.
  • Specifically describes how the diagnosis applies to the individual student.
Functional Limitations
  • Details how the disability impacts the student academically, including when and where limitations are likely to occur.
  • Describes the frequency, severity, and duration of limitations—especially important for conditions that are intermittent or cyclical.
  • Documentation must be recent enough to reflect the student’s current functional limitations.
  • Limitations must be supported by objective assessment data.
  • Requested accommodations must directly address the documented limitations.
